Trump Administration’s Medicaid Cuts Strip Coverage From Refugees and Other Legal Immigrants
Hundreds of thousands of refugees, asylees, trafficking-survivors and other legally resident immigrants awoke this week to find their Medicaid coverage terminated overnight. The change—quietly embedded in the Trump Administration’s 2025 tax cut and policy law—took effect on October 1 but only became visible on October 10 when state systems finished reconciling eligibility files with new federal guidance. According to the Congressional Budget Office, the broader Medicaid overhaul is expected to leave 7.5 million people without health insurance; immigrant groups fear they will be disproportionately affected. Under the revised rules, only immigrants who have had a “qualifying work history” of at least ten years—or who served in the U.S. military—remain eligible for federally-funded Medicaid. Most recent arrivals, including refugees still completing resettlement integration, no longer qualify. States may continue coverage with their own funds, but cash-strapped health departments in Texas, Florida and Arizona have already announced they will not pick up the tab. Hospital associations warn of rising uncompensated-care costs as disenrolled patients delay treatment until emergencies occur. Business travel managers and global-mobility teams say the move complicates short-term assignment planning. Multinationals that routinely sponsor humanitarian parolees or L-1 intracompany transferees now face higher private-insurance premiums and additional onboarding paperwork to ensure compliance with Affordable Care Act minimum-coverage rules. “The hidden cost of sending talent to the United States just went up,” noted one Fortune 100 mobility director whose company relocates about 400 foreign employees a year. Immigration advocates vow to challenge the policy in court, arguing that refugees are explicitly exempt from the five-year public-benefit bar under the 1996 welfare-reform statute. Several Democratic-led states are expected to file suit next week seeking an injunction before open-enrollment deadlines lock newly uninsured immigrants out of marketplace plans until fall 2027. Meanwhile, human-resources teams are advising affected employees to enroll in employer-sponsored coverage immediately—even if that means mid-year premium adjustments—to avoid gaps in care for chronic conditions. For companies, the practical takeaway is clear: budget extra for health benefits on U.S. assignments starting this quarter, update compliance checklists, and communicate early with assignees who may lose Medicaid before the holiday travel season.
